Policies

Session Frequency and Duration

The initial session is one hour long.  Subsequent sessions are up to 40 minutes depending on the work being done. Couples and family sessions are one hour. The frequency of the session is dependent on the type and goal of treatment.

Phone or video sessions are available when face-to-face meetings are not possible.

Fee

This will be discussed on initial contact. 

Payment

Payment is collected at the end of each session unless otherwise arranged. Check or cash is preferred but credit card is acceptable.

Insurance

Insurance is not accepted.  You are responsible for submitting your receipts to your insurance company for reimbursement depending on your out-of-network benefits for behavioral health. 

Receipts

A superbill can be mailed to you after each session for insurance purposes. This is not done routinely so please make it known that you require it.

Cancellation policy

Cancellation is required one business day in advance.  Missed appointments or appointments canceled on the same day will be charged.

Communications

Dr. Perkel prefers basic communication via email for medication refills and logisitics such as appointment requests and changes. However communications involving confidential information or more in-depth discussion should be done telephonically or in person.

Confidentiality

The practice complies with HIPAA (Health Insurance Portability and Accountability Act of 1996) regarding data privacy and record keeping safety. Any communication or release of any information to third parties can only be done with documented consent. However disclosures of information are mandated when there is a victim of abuse, when the patient or someone else is at acute risk for harm, or when court mandated. 

Medication prescriptions and refills

These requests can be communicated by email and will be responded to within 48 hours.  Dr. Perkel prefers to see you regularly to monitor medications. He will not refill prescriptions if he has not seen you in the past three months unless otherwise discussed. For controlled medications he must see you monthly.

Emergencies

In case of urgent matters please call Dr. Perkel's office at 347-855-4435.  In the event of an emergency please proceed to the nearest emergency room.
